I read with interest the article entitled, “Bariatric surgery before and after kidney
transplantation: a propensity score–matched analysis” [

]. In this article, the authors attempt to determine whether kidney transplant (KT)
is best performed before or after bariatric surgery (BS). The primary outcomes of
interest were patient and graft survival, and secondary outcomes were postoperative
complications and efficacy of weight loss. The authors studied 22 patients in the
KT first group and 34 in the BS first group. They found that death/uncensored graft
survival in the KT first group was significantly higher than in the BS first group
(90.9% versus 71.4%; P = .009), and there was no significant difference in 1-year total weight loss, 1-year
percentage of excess weight loss, or the incidence of postoperative complications
between the KT and BS first groups. The authors did not mention whether any of the
patients awaiting KT died while waiting for transplant or while waiting for a suitable
donor. The authors concluded that both pre- and post-transplant BS is effective and
safe and that each transplant candidate should be considered in detail to determine
